I think a little should be added describing how you will be using each one as well as an assessment as to the relative reliability of each one. The latter is particularly important; always consider how recent the source is, what sort of journal it appears is (is it a medical journal? A newspaper?) and who the authors are. As far as the formatting for the actual references, you have a solid grasp here. Do remember that they should be double-spaced, and remember the capitalization rules for article titles: only the first letter of the first word, the first letter after the colon and, of course, proper nouns or names (things that are always capitalized).
