Develop a plan of care for the patient of a specific spiritual/religious background. The patient has been diagnosed with a terminal illness and has been in your care for the past several days. On your last day with the patient, he/she dies. Please design a spiritual plan of care for this patient. Keep in mind this is a SPIRITUAL CARE PLAN. While some brief information about your patient’s physical condition may be helpful in understanding rationale for your care, it is not the focus of this plan. I do not want vital signs or nursing diagnoses. (Remember not all of the students in class are in the nursing program). This should be written out in paragraph form as outlined below, in the order described below. Be sure to include your spiritual assessment (summarized-do not include your questions) and make sure you address your patient’s spiritual needs. Describe how you will ensure your patient and the family’s spiritual needs will be met. In addition, include needs surrounding the last hours of life and the after death body care.
