Culture ChangeThe Artifacts of Culture Change is a tool created by CMA to assess and enhance performance of a long-term care facility. Your project will provide you with the opportunity to become familiar with this tool. You will create a fictitious Long-term care (LTC) facility for your project and use this facility as the basis to complete the Artifacts of Culture Change tool. Once you have completed the form, you will create an action plan for your LTC and focus on two areas to improve the facility.There are two parts to your project.Download and complete the Artifacts of Culture Change Tool. Create a fictitious long term care facility for your project. You will use this facility to base your project on and complete the Artifacts of Culture Change tool. You must select a minimum of two sections from the tool.submit:Completed Artifacts of Culture Change Tool – 2 sections (form)PowerPoint Presentation (with speaker notes)You are required to create speaker notes in your PowerPoint Presentation. The PowerPoint Presentation should be developed in a professional format that educates your audience (your instructor) on the facility.Presentation RequirementsYour presentation should be a minimum of 10 slides, including introduction and conclusion. You are required to include speaker notes in your PowerPoint Presentation. Your presentation, including the speaker notes should be free of spelling and grammar errors. Cite your sources used in your Presentation using APA citation. Your presentation (and speaker notes) must address the following information:The facility nameFacility sizeLocation of the facilityHistoric information (the age of the facility, type of ownership (for-profit, not-for-profit, or government organization) the number of beds, and the identification of the positions which compromise the management team and the management structure.A statement of findings – this should be based on the information in the Artifacts Culture Change Tool along with scores in each category and total scores.Recommendations – your recommendations should be based on your findings and limited to only ONE specific area (for example, reduction of high CAN turnover rate, increased involvement of family and community to create more home like atmosphere, etc.Action Plan – create an action plan for one specific category based on your recommendations. Note: The action plan is an important part of your project.Be sure to address each artifact in your presentation. Plagiarism will result in a failing grade.
